Although the bar ends are essentially the same, the heads of the E2 and the Equal-i-zer are quite different.

I think that's it, and it likely occurs if the brackets shift, or if they are installed in a less-than-ideal location due to interference with other stuff on the tongue (such as propane tanks). It's only likely when backing up, because the tug-trailer angle can get sharper when moving in reverse.
