That would fit under most bridges on highways in Canada, although the second story doesn't seem worth the hassle to me. A quick scan of a government list of hundreds of highway bridges in Alberta shows none on primary highways with less than 4.5 m (15 feet) of clearance.

It is safer to stay under the legally defined maximum vehicle height, which is 4.18 m (13' 8") here, and 4.15 m (13' 7") for commercial interprovincial traffic.
The U.S. DOT says

Lots of 13'6" clearances in our country down here. And a lot much shorter on the non interstate roads. Remember seeing a trailer near Omaha once where two guys were up in the trailer pulling the head and neck down on a giraffe so it would clear the overpass. They were headed for the Henry Doorley zoo in Omaha no doubt and had about 15 miles to go. Duck. No Giraffe.

Oh yeah, there are lots of campgrounds that are not good for tall RVs. Even some Escapes won't fit in Baxter State Park. Highways are another matter.

The popular "bus shell" choice for megadollar custom motorhomes is the Prevost H3-45 VIP; it is 149" tall overall - 12' 5" - and some people put air conditioners on top so they're well over 13' and hitting legal limits. That's not wildly tall for a large Class A motorhome. Still, the 15' tall trailer is silly...

So stupid, in so many ways.

The interior upper part seems to contain a set of stairs and a small hallway, so essentially nobody will ever hang out in that area. Its only purpose is to have the stairs that get to the roof. If it's nice enough for me to want to get out on the roof though, why can't I exit my trailer and go up an exterior ladder or a set of fold out stairs that can be stored neatly up against the rear of the trailer.

Once you eliminate the interior upper part, it's questionable if the upper railing is really worth the negatives in terms of height restriction. Lots of RVs have ladders that lead to the roof. If really skittish, you could always design railings that fold down when not in use.

Sure, fold down railings and folding ladders might be expensive to add, but adding the second level can't be cheap either.
