Although the CHP wouldn't tell me anything, our NBC affiliate evidently got a report at the scene:

"The California Highway Patrol says there are no indications that alcohol, drugs or distracted driving played a role in a wheel coming loose from a fifth-wheel trailer on Highway 101 in Paso Robles Thursday afternoon, killing a person in another vehicle.

At around 2:40 p.m., the 2014 Crossroads trailer was being towed by a Ford F150 truck when officers say the right wheel dislodged from the trailer’s rear axle and rolled across the southbound lanes."

Either the wheel was on the axle(and tire came off after all lugs detached) or not; seems it was not. That then sounds like a bearing failure. Which then makes one wonder whether the bearings were ever serviced.
