That looks like a push-though hub cover or cap, unusual only because it has a plated plastic centre cap (which is useful if you can take it out for access to the E-Z Lube hub). The wheel is aluminum alloy.
Given that the wheel nuts are centred on a 4.5" diameter circle, the centre bore (or pilot hole) appears to be about 3.5" in diameter... but you need a more precise value.

Does anyone know the diameter of the spare tire Escape uses? I want to buy a custom spare cover for our 21.

Wouldn't 15" be the diameter of the wheel?
Just had to know so I measured my spare ( 15 inch ) and diameter of wheel and tire is about 27 or 28 inches. It is dark and I had to hold the tape and the flashlight, so that's the best I can do.

The spare should be the same as the tires that are on the axles. A 21' uses the same tire as most recent Escapes, which is a ST205/75R15; although as I recall some trailers have come with different sizes, I don't think the 21' has had other sizes. It's easy enough to read the side of the tire and see what it is, anyway.
I agree that for cover clearance you need to know the overall diameter. The easiest source of this information is published specs, and I find TireRack.com the easiest source of specs. Overall diameter doesn't vary much by brand (within the same size designation), and a Goodyear Marathon ST205/75R15 has an overall diameter of 27.1"... just as Baglo measured.
A cover has a width limit as well, and the "205" is the nominal section width of the tire in millimetres. That's about 8 inches, as the specs show.
I would assume that a smaller tire is unlikely to be used on the trailer in the future, but it might be good to be able to accommodate a slightly larger tire just in case. The next common ST tire size up is ST225/75R15 (28.3" overall diameter).

I have the aluminium option for all five wheels on our 21' and was disappointed when I discovered that the spare didn't come with the center cover. I guess you are supposed to remove it from the flat and put it on the spare when you change tires. I didn't know how to do it when I discovered a flat while camping at Lake Louise and took the tire down to Canmore to be repaired. The dealer showed me how you simply give the hub cover a smack and it will pop loose out the back. When I get a little more ambition I will have to take off the replacement spare to put the hub on so all four wheels once again match.

As a helpful post, amazon.ca currently has the 27" Camco white spare tire covers (45346) on for a decent price ($12.xx) amazon.com was $10.xx when I looked. Both those prices beat the ebay ones that I found.

Is there any reason the Amazon #45322 two-pack at $16.73 wouldn't be the preferred choice?
P.S. Yet another reason why the FedEx UPS and USPS trucks wait in line for parking in front of my house.

Since we are on wheels.....could anyone tell me what size lug nuts are on the wheels? I am going to purchase a torque wrench and socket to make sure I have the wheels safe and sound. Thanks.

LOL - one day as my UPS guy was bringing me yet another Amazon package, he said, "I love Amazon, we were in the middle of big (wage/benefit) negotiations and Amazon just said to UPS - settle it" I don't know how accurate he was in all that but UPS sure gets a lot of business from Amazon. Of course, the other side of the coin, are my friends who say Amazon is going to put everybody else out of business.
But for us, living out in the sticks, it sure save us a lot of time and mileage vs driving long distances to make purchases.
